Online Incident and
Crisis Forms
The Situation Center
Standardize information gathered during a response
Electronic forms are a valuable tool to use during a response. They eliminate uncertainty about what information to capture and who needs to see it. They also serve as valuable regulatory and compliance documentation.
Paper forms can be misplaced and might not be accessible to everyone who needs them, particularly when team members are in different locations. Even electronic forms can be overlooked when they're stored in different applications. These problems can lead to delays—or worse.
A better way to collect and manage incident data
MissionMode's Situation Center gives you a single place to complete, manage and complete forms used for a crisis response—all within a simple-to-use incident management system that's been proven effective during all types of incidents.
Situation Center Forms standardize the information that's gathered, minimizing error and ensuring that no details are missed. Forms are processed faster and important information flows to the right people.
Electronic forms are also a perfect complement to the Situation Center's incident log, which captures free-form data and file attachments.

How it works
Forms are built in an easy-to-use visual forms designer. Drag and drop fields such as text, drop-down lists, checkboxes, date selectors, and more.
When a form is completed, it automatically alerts the people who need to process or view the completed form. Authorized users track the status of the form and access information when needed. And, the form becomes a part of the incident management record.
Flexible forms creation
Build a wide range of forms such as regulatory reports and forms unique to your organization. To further standardize forms data, define assets and asset types that are used in form selections—equipment, people, facilities and any type of data.
